Provo Entry Forms

The entry forms for the 2013 Provo High Dancesport Championships are available. You can download them below. Please note that late fees have changed for UBEA competitions and are detailed below:
- Entries are due by January 11 to avoid a late fee.
- After January 11, each entry form will have a $10 late fee.
- After January 18, each entry form will have a $20 late fee.
- After January 26, no late entries will be accepted if they create an additional round.
Here are the entry forms:
- Amateur Entry Form (Pre-Teen, Junior, Youth, Adult)
- Syllabus Entry Form (PeeWee, Elementary, Junior High, High School)
- Team Entry Form (Medley and Showdance)
